NorthStar Care Community is a nationwide organization of mission-driven providers, programs and services devoted to continually increasing access to care for serious illnesses and support during end of life – especially at home. Unfortunately, many families don’t know about available services, often at little or no cost to them – services that manage symptoms, provide emotional and spiritual counseling, and increase comfort. Our members combine decades of expertise to connect patients and families with the personalized care they need.
